I built this application to easily capture a client's scores when conducting a functional movement screen assessment as defined by FMS. Additional features include the ability to manage multiple clients, update a client's scores during retesting and include notes for any specific screen.

FMS Reference
This is a quick reference on how to score each screen.
Deep Squat (overhead)
- Score 3
- hip lower than knee
- arms remain locked out, dowel over feet
- knees and feet stay straight
- Score 2
- same as 3, but heels lifted
- Score 1
- w/ heels lifted, any one fault of 3
Hurdle Step
- Score 3
- hips, knees, ankle remain in align
- dowel remains parallel to hurdle
- little to no lumbar movement
- Score 2
- any one fault of 3
- Score 1
- cannot perform movement
- hit hurdle
- lose balance
Inline Lunge
- Score 3
- back knee taps board
- dowel remains vertical
- dowel keeps contact w/ head, mid-back and sacrum
- Score 2
- any one fault of 3
- Score 1
- cannot perform movement
- cannot get into position
- lose balance
Active Straight Leg Raise
- Score 3
- ankle falls between ASIS and mid-thigh
- Score 2
- ankle falls between mid-thigh and mid-knee cap
- Score 1
- ankle falls between below mid-knee cap
Shoulder Mobility
- Score 3
- hands w/in 1 hand length apart
- Score 2
- hands w/in 1.5 hand length apart
- Score 1
- hands greater than 1 hand length apart
Trunk Stability Push-up
- MEN
- Score 3: performed push-up, no sag, hands at forehead
- Score 2: performed push-up, no sag, hands at chin
- Score 1: unable to perform a rep w/ hands at chin
- WOMEN
- Score 3: performed push-up, no sag, hands at chin
- Score 2: performed push-up, no sag, hands at shoulder
- Score 1: unable to perform a rep w/ hands at shoulders
Rotary Stability
- Score 3
- same side rep complete
- Score 2
- opposite side rep complete
- Score 1
- cannot complete opposite side rep
